Insurance

We are currently in-network with Anthem/Blue Cross Blue Shield, TRICARE, Aetna, Medicaid, Amerigroup, Peach State Health Plan, and CareSource.

We also accept Private Pay. Payment in full is due for all services at the time they are rendered.

 If we are not in-network with your provider or are in the approval process, we can provide you with a signed itemized receipt that you may submit to your insurance provider for reimbursement.

Please remember to bring in a PHYSICAL copy of your insurance card for each appointment. Thank you!
