I am currently in-network with Anthem/Blue Cross Blue Shield, TRICARE, Medicaid, Amerigroup, Peach State Health Plan, and CareSource.

I also accept Private Pay. Payment in full is due for all services at the time they are rendered.

 If we are not in-network with your provider or are in the approval process, I can provide you with a signed itemized receipt that you may submit to your insurance provider for reimbursement.

Please remember to bring in a PHYSICAL copy of your insurance card for each appointment. Thank you!
